a) Sworn affidavit from the Management with reference to the infrastructure, faculty, and equipment regarding the Hospital for clinical affiliation and about other facilities as per the norms.
b) Sworn affidavit from the faculty members regarding their employment in the institution
c) Attested photographs with regard to availability of the building and facilities like class rooms, labs.
d) APPMB inspection proformas of the course to be filled by inspector.
e) The inspection reports forwarded by The Principal should reach
“The Secretary, A.P.Paramedical Board, O/o. the Director of Medical Education, Old Govt.General Hospital Campus, Hanumanpet, Vijayawada-520003".
f) Inspection Proforma for each subject are displayed on thewww.appmb.co.in
TA/ DA :
1. Sitting Fee Rs.1000 per institute
2. D.A. Full day / Half Day – Rs. 800/400
3. Travelling Allowance – Rs. 12/- per kilometre point to point / Second AC fare by train journey
